You will need form SC7 (for birth) or SC8 (for adoption) to hand to complete the box. Your employee will have completed and given this to you when they applied for Additional Statutory Paternity Pay.

You can only receive payment for the same number of weeks that Statutory Maternity Pay, Statutory Adoption Pay or Maternity Allowance remains unclaimed by your employee’s partner. There has to be a minimum of 2 weeks remaining unclaimed.
